Job Title: Case Manager
Percent Time: 100%
Hours: Flexible, Full Time
Location: UC Davis Medical Center, Sacramento, CA
Job Summary:
The Case Manager for the Wraparound Project works at UC Davis Medical Center's Trauma Service with youth and young adults who have been injured by violence. He/she will provide case management and provide links for individuals to important resources in order to reduce risk of re-injury. When patients are identified as potential participants, the Case Manager will screen for enrollment, and play the role of mentor of study participants, connecting them to services and resources after discharge and continuing to provide case management for up to twelve months.
Responsibilities:
I. Enrollment of participants
- Interview potential youth and administer the Violent Injury Screening Instrument to all participants in the hospital using cultural sensitivity
- Enter data in the database
II. Case Management/Mentorship for Wraparound Participants
- Connect participants with resources in the community to decrease behavior associated with violent behavior, including training and job opportunities, family services, anger management and violence prevention organizations in the community.
- Organize and update comprehensive list of community resources
- Conduct second interview at 6 months and 12 month after the injury.
III. Meetings/community organizing
- Attend established community meetings regarding violence prevention as required.
Qualifications:
Required:
- - High school diploma/GED
- - Cultural sensitivity and excellent people skills
- - 5 years experience working with youth services
- - Ability to work independently and as part of a multi-disciplinary team
- - Knowledge of resources for at-risk youth
- - Ability to have flexible working hours
- - Driver's license
Preferred:
- - Spanish speaking
- - Experience working with violence prevention programs
Salary Range: $46,536 - 55,620
